Dermal filler aftercare checklist for the first 48 hours and 7 days

The single most important rule after dermal filler is this: keep the area calm. No heat, no hard workouts, no alcohol, and no rubbing or massaging for the first 24 to 48 hours. I do dermal filler appointments in Lihue all the time, and this one habit is what protects that “refreshed, natural” look you came in for. If you’re treated at Kukui Grove and you’re heading back into our warm, humid island weather, this matters even more.

First 24 to 48 hours after dermal filler: the do’s that keep swelling down

Dermal filler is a 60-minute appointment for most areas, but your first two days are where you protect your result. Most people do great with minimal downtime, but bruising and swelling are still normal. Here’s what I generally recommend right away.

Do this

  • Cold compress in short rounds (think 10 minutes on, 10 minutes off) for swelling.
  • Sleep on your back with your head elevated the first night if you can.
  • Keep your face clean and hands off the treated area. Gentle is the name of the game.
  • If you bruise easily, plan for a low-key couple days. A lot of my Kukui Grove clients schedule mid-week so they’re not trying to “hide a bruise” at weekend plans.

Avoid this

  • Heat: hot showers, saunas, steam rooms, hot yoga, and long sun exposure.
  • Heavy exercise: intense cardio and heavy lifting. (More specifics on workout timing below.)
  • Alcohol for the first 24 hours since it can worsen swelling and bruising.
  • Blood-thinning meds/supplements unless your prescribing doctor told you otherwise. If you’re unsure, ask me before you take anything.
  • Touching or massaging the area, including facial massage tools. Don’t “check it” all day in the mirror.

Makeup is a big one. If you had injection points, I prefer you wait until the next day before applying makeup directly over those spots. And if you have to wear something, keep it light and use clean brushes or a fresh sponge.

Days 2 to 7: the dermal filler swelling timeline and what to avoid in Hawaii

Most swelling happens in the first 48 hours, then settles gradually. Bruising can take a bit longer. Lips, in particular, often swell more than cheeks or chin. It’s common to feel small firm areas or unevenness early on. That usually softens as swelling goes down and the product settles.

Sun exposure, sweat, and heat

In Lihue, the sun and humidity are real. Strong sun and heat can keep you puffy longer. Keep workouts lighter until you’re past that initial swelling window, and be strict about SPF. If you’re outside for work, reapply sunscreen and use a hat.

Swimming in the ocean or pool

I generally recommend waiting at least 48 hours before ocean or pool time. Saltwater and chlorinated water can irritate injection sites, and the sun and heat that usually come with swim days can prolong swelling. If you do swim after that window, keep your face out of the water as much as you can, rinse after, and keep the area clean.

Flying and pressure changes

Most people can fly after filler, but if you know you swell easily, try to schedule injections a few days before travel. That way you’re not dealing with puffiness on day one of your trip. If you’re coming in from Kapaa, Koloa, or Wailua and you’ve got travel plans, tell me and we’ll pick timing that makes sense.

Simple products and habits that protect your dermal filler results

After filler, your skin doesn’t need a complicated routine. It needs calm. Keep things gentle for the week, especially if you’re prone to redness or sensitivity.

My go-to aftercare basics: a gentle cleanser, a plain moisturizer, and a mineral SPF you’ll actually reapply. Skip strong acids, retinoids, and aggressive exfoliation over the treated area for several days, or longer if you’re still tender.

Facials, massage, and dental work timing

Hold off on facial treatments and facial massage for at least two weeks unless I tell you otherwise. The same goes for using at-home devices that press, heat, or pull on the skin. Dental cleanings and big dental work are best scheduled away from your filler appointment when possible. If you’ve got something already on the calendar, let me know so I can guide you on timing.

Gentle touch, but only after the early window

After the first 48 hours, light, gentle cleansing is fine. If I specifically recommend a very gentle touch for swelling, I’ll show you exactly how. Don’t do deep rubbing, and don’t try to “mold” the filler on your own.

When to reach out to me in Lihue (normal vs red flags)

A little swelling, tenderness, and bruising are common. Mild lumps early on can be normal too, especially in the lips. What I don’t want you doing is waiting it out if something feels off. You’re not bothering me. You’re protecting your face.

Usually normal

  • Swelling that peaks in the first 24 to 48 hours, then improves
  • Bruising that fades over several days
  • Tenderness or a tight feeling when you move your mouth (common after lip filler)
  • Mild asymmetry early on while swelling settles

Call me immediately

  • Severe pain or pain that’s getting worse quickly
  • Skin discoloration or blanching (white, gray, or mottled skin) near the treated area
  • Rapidly worsening swelling, especially if it’s one-sided
  • Spreading redness, warmth, fever, or drainage that could signal infection

If you’re in Lihue, Puhi, Hanamaulu, Wailua, Kapaa, or Koloa, quick follow-up is easy. And if you were treated near Kukui Grove and you’re stepping right back into work, I’ll help you plan timing so you look presentable while things settle.

If you want a bigger picture of injectables I do here, you can read Injectable Neurotoxins, Fillers & Collagen Stimulation in Lihue, or check the Kukui Grove-focused posts: Injectable Neurotoxins, Fillers & Collagen Stimulation at Kukui Grove and the Kukui Grove guide to injectables.

Quick reminder: Dermal filler is starting at $450. If you’re not sure what area makes sense for your goals, I’ll walk you through options during your consultation and keep the plan conservative and natural-looking.

Save this 7-day checklist (and keep it on your phone)

  • Day 0 (today): cold compress, no heat, no alcohol, no workouts, no rubbing, keep it clean.
  • Night 1: sleep elevated and on your back if possible.
  • Days 1 to 2: skip saunas, hot yoga, intense cardio, and facial treatments. Makeup over injection points waits until next day.
  • Days 2 to 7: be strict with SPF, keep workouts moderate, don’t pick at skin, keep products gentle.
  • Anytime: call me for severe pain, rapidly worsening swelling, spreading redness, or skin discoloration/blanching.
